Early Class III & Underbite Treatment

Underbite Treatment for Children in Anaheim Hills, Yorba Linda, Orange & Placentia

If your child's lower teeth sit in front of the upper teeth, your child may have an underbite, reverse bite, or Class III malocclusion.

Certain underbites are best evaluated while a child is still growing. At My Orange County Orthodontist , we evaluate the teeth, jaws, facial growth, bite and oral function together to determine whether early orthodontic treatment may benefit your child.

Understanding Class III Growth

What Is an Underbite in a Child?

An underbite occurs when some or all of the lower front teeth are positioned in front of the upper front teeth when the child bites together. Orthodontists may also describe this as an anterior crossbite, reverse bite, or Class III bite.

The appearance can look similar from child to child, but the underlying cause may be very different.

In some children, the upper jaw may be positioned too far back. In others, the lower jaw may be developing more prominently. Some children have a combination of dental and skeletal factors.

That is why the first step is not simply deciding whether to use braces. The first step is determining why the underbite is developing.

Real Early Treatment Results

Class III Treatment Is About More Than Straightening Teeth

Early Class III treatment may allow an orthodontist to improve the bite during a period when the facial bones are still growing.

Why Timing Matters

What Are the Advantages of Treating an Underbite Early?

Not every child with an underbite needs immediate treatment. However, certain Class III problems are worth evaluating early because the treatment options available during growth are different from those available after facial growth is complete.

01

Work With Your Child's Growth

A growing child may provide an opportunity to influence the relationship between the upper and lower jaws while growth is still occurring.

02

Correct the Reverse Bite

Bringing the upper and lower teeth into a more favorable relationship can improve the developing bite and function.

03

Address Maxillary Deficiency

In selected children whose upper jaw is positioned behind the lower jaw, orthopedic treatment may be used to encourage a more favorable maxillary relationship.

04

Monitor Growth Earlier

Class III growth can continue during adolescence. Starting the relationship early allows us to monitor changes carefully as your child matures.

05

Watch Permanent Teeth Erupt

We continue evaluating the bite as permanent teeth erupt so developing problems can be recognized before they become more difficult.

06

Plan for the Long Term

Early treatment is not just about today's result. We continue monitoring the child's facial growth and bite over time.

Growth Modification

RPE and Facemask Treatment for a Child's Underbite

One treatment approach for selected growing patients with a Class III bite and maxillary deficiency is an orthodontic facemask, sometimes combined with a Rapid Palatal Expander (RPE).

When expansion is indicated, the RPE helps address transverse development of the upper arch. The facemask then applies forward-directed orthopedic forces to the maxillary complex.

Not every underbite requires an expander or facemask. Treatment depends on the child's facial pattern, age, growth, jaw relationships, dental eruption and individual diagnosis.

Crossbite & Underbite Treatment

Every Class III Bite Is Different

Some children have an underbite together with a posterior crossbite, narrow upper arch, crowding or other developing orthodontic concerns.

These problems should be evaluated together rather than treating only the front teeth.

Class III and crossbite treatment should be based on the child's complete bite and growth pattern rather than only the position of the front teeth.

Oral Function Matters

We Also Look at Tongue Posture

Orthodontic treatment is not only about teeth and appliances. We also evaluate the environment around the teeth, including tongue posture, swallowing patterns and oral habits.

The tongue is a powerful muscular structure that interacts continuously with the teeth and dental arches.

When abnormal tongue posture or swallowing patterns are present, those forces may contribute to unfavorable tooth position or affect orthodontic stability.

We therefore pay attention to tongue position and oral function when evaluating children with developing bite problems.

It is important to understand that Class III growth is complex. Tongue posture alone does not explain every underbite, and continued mandibular growth may occur even after successful early correction.

Long-Term Orthodontic Monitoring

What Happens After the Underbite Is Corrected?

This is an important question because Class III treatment does not end the day the bite is corrected.

Children continue growing.

A child may have a beautiful early correction and still experience changes in the relationship between the upper and lower jaws as adolescence progresses.

For this reason, we continue to monitor:

Jaw growth
Overjet and bite relationship
Permanent tooth eruption
Facial development
Tongue posture and oral habits
Long-term bite stability

Our goal is not simply to correct what we see today. We want to follow the child's development and recognize changes early if additional treatment becomes appropriate.

Parent Questions About Underbites

Frequently Asked Questions About Child Underbite Treatment

What age should a child's underbite be treated?

There is no single ideal age for every child. Treatment timing depends on the child's growth, whether the problem is primarily dental or skeletal, which jaw is contributing to the discrepancy, and the stage of dental development. An obvious reverse bite is worth evaluating early so an orthodontist can determine whether treatment or observation is appropriate.

Should I wait until all of my child's permanent teeth come in before treating an underbite?

Not necessarily. Certain Class III problems are evaluated while a child is still growing because growth modification options are different after facial growth is complete. An orthodontic evaluation does not automatically mean treatment will begin immediately.

Can a child's underbite be treated without surgery?

Many growing children with underbites can be managed orthodontically or orthopedically depending on the cause and severity of the problem. More severe skeletal discrepancies may still require additional treatment later because future facial growth cannot be predicted with certainty.

What is a Class III bite?

Class III describes a bite relationship in which the lower dental arch or lower jaw is positioned relatively forward compared with the upper arch. Parents often recognize it because the lower front teeth appear in front of the upper front teeth.

What does an orthodontic facemask do for an underbite?

In selected growing patients, an orthodontic facemask applies forward-directed orthopedic forces to the upper jaw. It may be considered when the Class III relationship is associated with a relatively retrusive or deficient maxilla.

Why is an expander used with a facemask?

An expander may be used when expansion of the upper arch is clinically indicated. Depending on the diagnosis, an RPE may be combined with facemask therapy as part of a broader Class III treatment plan.

Does every child with an underbite need a facemask?

No. Underbites can result from different combinations of dental position, upper-jaw development, lower-jaw growth and facial pattern. Treatment should be based on diagnosis rather than simply the appearance of the front teeth.

Can an underbite return after early orthodontic treatment?

Changes can occur as a child continues growing, especially in patients with a strong Class III growth pattern. This is one reason continued orthodontic monitoring throughout growth is important even after an excellent early correction.

Can tongue posture affect orthodontic treatment or bite stability?

Tongue posture and swallowing patterns are part of the muscular environment surrounding the teeth. Abnormal function may contribute to unfavorable dental forces in some patients and may be relevant when considering long-term orthodontic stability.

Why do you monitor children after early underbite treatment?

Facial and mandibular growth can continue for years after an early orthodontic correction. We monitor the developing bite, permanent teeth, facial growth, jaw relationship and oral function so important changes can be recognized as early as possible.
